C Change

C Change, a program within the Health Federation of Philadelphia,  works to advance the goals of hepatitis C virus (HCV) elimination through training, practice facilitation, and capacity building interventions designed to improve the HCV care continuum. Our approach is collaborative, tailored to the unique needs of each setting, and prioritizes the creation of low threshold access to HCV testing, prevention and care.

What We Do

C Change collaborates with settings serving priority populations to optimize workflows, implement best practices, and remove barriers to hepatitis C care. Our approach starts with identifying the organization’s goals, followed by an in-depth capacity assessment and collaborative implementation planning process involving staff at all levels. We work closely with key members of the organization to identify and implement necessary training and capacity building interventions to achieve the organization’s goals and integrate various stages of the HCV care cascade—testing, linkage to care, and treatment. 

What We've Done

  • Increased HCV treater capacity within syringe service programs, FQHCs, and SUD treatment settings through training, clinical mentorship, and on-site TA
  • Implemented standard of care HCV screenings and testing in multiple settings and levels of care, including syringe service programs, FQHCs, mobile care providers, and SUD treatment programs.
  • Convened Medicaid physical and behavioral health payers to resolve barriers to integrated SUD/HCV care created by payer silos.Brokered relationships and partnerships between medical providers and SUD treatment programs to increase access to testing and care for communities most impacted by HCV.
  • Delivered over 100 viral hepatitis trainings to substance use disorder (SUD) treatment programs treatment programs, Federally Qualified Health Centers (FQHCs), Single County Authorities (SCA), Medicaid payer networks, and others
  • Created tailored patient education and best practice guidance materials which can be accessed at stophiv.com/sor
