Community-Based Organizations Come Together for Interactive Connect4Health Event

The Health Federation of Philadelphia, in partnership with HealthShare Exchange (HSX), held an event recently on the campus of Temple University for community-based and other social service organizations to learn more about how to utilize Connect4health.org.

Powered by findhelp, Connect4Health is a free online resource and referral platform that helps organizations increase their program's visibility, gather referral data, track referral activities, and find new resources. 

The half-day workshop included a platform demonstration, explaining how to search for programs and send referrals; a session on how to manage program listings and gain access to free CBO tools, exclusively available to “claimed programs”; and a question and answer session with several community-based organizations that currently take advantage of what Connect4Health offers. Those in attendance were also given the chance to network throughout the day, building relationships that will stretch far and wide.

The workshop concluded with presentations from Bill Marella, Vice President of Value-Based Care and Analytics at HSX, and Beck Moore, CEO of Community Action Association of Pennsylvania (CAAP). Bill discussed PA Navigate, Pennsylvania’s vision for bridging healthcare and social care in Pennsylvania, and how community-based organizations can participate and benefit. Beck spoke about CAAP’s upcoming efforts to engage community-based organizations around the state with the PA Navigate tool.
