This month, we spoke with Tiffany Johnson, Hub Practice Manager at Project Home — Hub of Hope/Stephen Klein Wellness Center

What type of services/programs does Hub of Hope offer? 

The Hub of Hope (HUB) is a safe place where people who are experiencing homelessness can take a shower, wash their laundry, have coffee, or speak to peers or resource coordinators for emergency housing. In addition to social services, the Hub of Hope offers a range of health care services, including primary medical care, behavioral health care, and reproductive health services. Our services include a psychiatric nurse practitioner, whose office hours are  Tuesday through Thursday. Her schedule books quickly! We also have an occupational therapist from Jefferson Hospital onsite four days a week who can assist with resumes, among other things. In addition, we have a Medication Assistance Treatment group, which meets on Tuesdays. And we have our Behavioral Health Consultants, who educate staff and employees about abuse, trauma, social care, and other behavioral health issues.

We offer same-day courier service from the Stephen Klein Wellness Center pharmacy for our uninsured patients. This service is available for patients seen in the early morning (for example, picking up medications at 2:30 p.m. for morning appointments). The HUB also offers transportation assistance advocacy (bus passes) during medical appointments for physically or mentally challenged patients (such as stroke and dementia patients and cancer survivors). 

Who are these services for, and are there any eligibility requirements? 

These services are primarily for adults who are either uninsured or who have Medicaid or Medicare. When children or pregnant people come into the HUB, we’ll initially serve them for the immediate need they have and then transfer them to be seen by clinical staff at the Stephen Klein Wellness Center. We will see any uninsured or state-covered adult, whether they have documentation or not. We do not ask about immigration status at HUB. 

What is the best way to connect to your programs? 

Call 215-309-5225 OR visit us between the hours of 8 a.m. to 3 p.m. Call us if you can't find the space. We know it can be confusing underground and often hard to find signage pointing the right way.

What’s one resource you have found to be super helpful for your patients/clients?

We have an external partnership with the Birth Certificate Clinic. The Homeless Advocacy Project helps our patients get that document for free. This is a big first step in getting documentation necessary to apply for benefits and aid.
